Job Responsibilities
- The Physical Therapist is responsible for providing rehabilitation treatment and care in the home to the geriatric population.
- Evaluates patients and develops a rehabilitation plan within the parameters prescribed by the referring physician to improve or restore physical functioning, alleviate pain and prevent physical dysfunction.
- Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the Physician, DON, and Therapy Services Manager, according to policy.
- Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff.
- Documents patient progress and communicates with other clinicians such as occupational therapy, speech therapy and skilled nursing on the status of patient's treatment plans.
- Participate in the agency’s quality improvement program.
- Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
- Directs and supervises Physical Therapy Assistants on the course of treatment for designated patients.
- Educates and instructs patients, families, and caregivers regarding treatment plans and procedures.
- Plans patient discharges and communicates proper follow-up care to patient and caregivers.
- Adheres to established organizational and departmental policies, State and CMS guidelines, standard operating procedures, quality assessment, and safety guidelines.
- Displays and maintains behavior which promotes a positive experience for patients, caregivers, referring physicians, and co-workers.
Education, Licensure(s), And Qualifications.
- Experienced Physical Therapist is preferred but not required. New grads welcome to apply!
- Previous Home Health experience is preferred but not required.
- Must possess attention to detail skills.
- Ability to prioritize work using fundamental time management skills.
- Must possess excellent interpersonal skills to foster a team environment.
- Demonstrates knowledge and technical skills necessary for providing competent, comprehensive patient care.
- Must possess problem solving abilities, excellent customer service skills, and patient first characteristics.
- Ability to perform essential job functions with or without reasonable accommodation.
- Must be currently licensed to practice in the state of Indiana.
- Must be CPR certified or willingness to acquire before start date.
- Must be a graduate of a Physical Therapy Program accredited by the American Physical Therapy Association (APTA).
- Current Drivers License, vehicle insurance, and access to a dependable vehicle.
